Public vs. private health insurance in Germany: how to actually decide

The choice most newcomers make in their first month — and the one that's hardest to reverse later. Here's what actually matters.

In Germany, almost everyone has statutory (public) health insurance by default. Above a certain income threshold, you can opt into private insurance instead — a decision that shapes your costs, coverage, and flexibility for years.

Both systems cover the essentials. The real differences show up in cost predictability, dependent coverage, and how difficult the choice is to reverse later.

The two systems, side by side

Public (GKV)

Cost scales with income. Family members covered for free. Easy to keep long-term. Fewer extras, more predictability.

Private (PKV)

Cost based on age and health at signup. Broader coverage options. Each family member pays separately. Harder to reverse later.
